On Tuesday, a boiler exploded at GlaxoSmithKline's Rixensart, Belgium manufacturing site, resulting in the death of a production technician.
The fuel boiler exploded during the maintenance process of several boilers located outside the production area. A building door was blown off and debris scattered, striking a 24-year old production technician who later died from his injuries.
An investigation was started Wednesday morning to determine the cause of the accident and if anyone was responsible.
Headquartered in Belgium, GSK Vaccines operates from three sites: Rixensart, Wavre and Gembloux. Last year, the same Rixensart site accidentally dumped 45 liters of liquid contaminated with poliovirus into the local sewage treatment plant, and subsequently into the Lasne River.
